Visa Application Form

Requirements Checklist
1. Download the Visa Application Form below.
2. Complete the online pdf form making sure you click on the Submit button to submit the data to our system.
4. Print a copy of the confirmation page that appears after you successfully submit the form and give this together with your application form.
3. Submit a printed copy of your completed form together with one (1) recent passport-sized photo and the appropriate visa fee at least fifteen (15) days before intended date of travel.
4. Business/residential addresses and contact telephone numbers to be provided.
5. Occupation of applicant to be specified.
6. Financial means to be indicated.
7. Proposed date of travel to be stated.
8. Names and addresses of references in Ghana to be provided.
9. Letter of consent needed from parent/legal guardian for persons under 18 years.
10. Copy of Biodata page of applicant's passport

The visa application process only commences upon receipt of the hard copy of your visa application form, accompanied by all required supporting documents to the Mission, either at the counter or by post.

Passport Application Form

Requirements Checklist

1. Four (4) identical passport-sized photos.
2. Proof of Ghanaian citizenship (originals only).
3. Documents to support identity (originals only).
4. Previous Ghanaian passport, if necessary.
5. For New Borns in Belgium please add Original Birth Certificate (translated if not in English)
6. Processing fee.


Lost, Damaged or Stolen Passports
Applicants must produce a Police Report, a birth certificate, an affidavit and Proof of Identity.

Change of Name
This must be supported by duly-authenticated documents.

Citizens Registration Form

It is in the interest of all Ghanaians resident in Belgium to register with the Ghana Embassy, Brussels. This will help the Mission to provide the necessary assistance and protection to such persons in case of emergency. Changes of address and departure from Belgium should also be notified to the Mission.
